Registering as a new patient

We are glad you'd like to become a patient at Tandartspraktijk Arrindell! You can register online using our registration form.

This is how it works

1. Fill in the form. Using the button above, you can open a digital form for you to complete. Please have your BSN (citizen service number) and insurance details handy
2. We'll get in touch. We will call you as soon as possible to schedule your first appointment.
3. Initial consultation. During your first visit, we get to know you, examine your teeth, and discuss your needs.

Please bring to your first appointment

• Valid proof of identity (also for children)
• Your health insurance card
• An up-to-date medication overview (ask your pharmacist for this)

Patient card from your previous dentist

Were you a previous patient of another dentist? Then ask your previous dentist to send your patient card to us, or email it to him yourself via secure email.
